Robotic plication for paralysed (raised) hemi - diaphragm

Robotic repair of ruptured diaphragm

Traditionally, this operation was performed through an open thoracotomy and in modern times, advances in minimally invasive surgical technologies have allowed diaphragm plication to be performed with much less surgical trauma to allow for quicker recovery. Robotic plication of the diaphragm represent the most modern surgical approach.

Three small incision of 8mm each are necessary to perform this procedure and the patient is usually discharged within 48 hours of admission. The discomfort from the incision remains minimum and patient can quickly return to his normal activities.

Below you can see the plication of the diaphragm performed with running sutures across the paralysed diaphragm and the positioning of the robot on the lateral side of the patient.

Robotic Thoracic Endometriosis Surgery

Thoracic endometriosis is a rare condition where endometrial tissue—normally found in the uterus—grows inside the chest cavity, often affecting the diaphragm, pleura, or lungs. It can cause chest pain, shortness of breath, coughing up blood (hemoptysis), and symptoms that worsen during menstruation.

Robotic thoracic surgery is a highly effective, minimally invasive technique used to diagnose and treat thoracic endometriosis. Using the Da Vinci robotic system, the surgeon can access hard-to-reach areas within the chest through small incisions, allowing precise removal or ablation of endometrial implants. This approach offers improved visualisation, enhanced dexterity, and reduced trauma to surrounding tissues.

Benefits of robotic surgery include:

  • Accurate identification and removal of endometriotic lesions

  • Minimal postoperative pain and scarring

  • Faster recovery and shorter hospital stays

  • Lower complication rates compared to traditional open surgery

Robotic thoracic endometriosis surgery is often performed in collaboration with gynaecologists as part of a multidisciplinary approach, especially when pelvic endometriosis is also present.

Mr Nikolaos Panagiotopoulos is one of the few thoracic surgeons in the UK with experience in managing thoracic endometriosis robotically, offering expert, patient-centred care for this complex and often misdiagnosed condition.
